Better Birth Outcomes, Chicago Family Case Management, Healthy Families Illinois and Healthy Start. Within these four programs, our goals are to:

• Help patients identify a medical home

• Improve women's health before, during, and after pregnancy

• Prevent infant mortality and morbidity

• Help families care for their newborns and infants

• Reduced parental stress and incidents of child abuse and/or neglect

• Strengthen mother-child interaction

• Ensure that infants and children are up-to-date with immunizations and well-child visits

• Provide prenatal and parental education

• Build relationships and help clients to identify personal networks of support through home visiting

• Work collaboratively with community partners to enhance supportive care and referrals within the clients' home communities

Our Community Action Network (CAN) is made up of participants, community partners, and staff.  It is our goal to engage members in collective organizing and activities to address the intersections affecting the health outcomes of our guests.  Our CAN's purpose is to identify priorities that address reducing barriers, ensuring access to care, and supporting families in reducing infant mortality.

 

Near North’s Maternal and Child Health program supports and promotes the health and well-being of women, children and families.  Staff strives to engage women who are at risk in prenatal health care early in their pregnancy and provide ongoing care, education and support throughout their pregnancy and postpartum. In addition to providing high-risk obstetric services, the program also provides critical screenings, counseling and referrals for drug and alcohol dependency, domestic violence, and mental health issues that impact the health of both the mother and child.

 

The program also provides peer support and leadership training, which helps to empower women and build a sense of community. Our case managers are a trusted source of guidance and support.
