Scheduling Your Appointment
To make an appointment at any of health centers, call 312-337-1073, press option 2 and one of our dedicated staff members will be able to schedule you. Same Day Access appointments are also available.
If you are a current patient, please arrive 15 – 20 minutes before your appointment time for register. If you are a new patient, please arrive at least 30 minutes before your appointment time for registration.
If you are insured, please bring:
1.) Identification
Identification is required. We accept one or more of the following types of identification:
• Driver’s License
• State ID
• Employment ID
• Public Aid ID
• Utility Bill indicating name and address of patient, parent or guardian
2.) Proof of Insurance
If you are uninsured, please also bring:
1.) Identification (same as above).
2.) Verification of Income
If employed, two recent check stubs or W2 Tax Statements.
If unemployed, Proof of Income (Weekly, Biweekly, or Yearly): SSI benefits, Unemployment benefits, Disability Benefits, Child Support payments with copy of child support agreement.
If you don’t receive benefits, bring a notarized letter of support or non support.
