Our "Tree of Life" Campaign Promises to Bring
Healing, Health and Hope to Chicago's Disadvantaged!
Dear Friend of Near North Health Service Corporation:

Excitement is filling the air at Near North Health Service Corporation as we continue providing primary healthcare services to men, women and children in Chicago. As our expansion plans are taking shape, we would like to invite you to join us in helping low-income Chicagoans and the communities where they live by participating in our 2007-2008 “Tree of Life Sponsorship Program”.

Near North Health Service Corporation needs your support to help continue to provide quality healthcare services to the less fortunate. Our “Tree of Life Sponsorship Program” allows individuals, small businesses, corporations, churches and community groups to give to a very worthwhile cause while receiving major visibility. All leaves and acorns will be prominently displayed on our “Tree of Life” sponsorship wall, which will be located at our corporate office. Leaf and acorn designations are as follows:

Acorn Member: (up to $99)
Honorable mention in all NNHC print materials, a plaque on our
“Tree of Life” and an Acorn

Bronze Leaf Member:
($100-$250)
Honorable mention in all NNHC print materials, a plaque on our
“Tree of Life” and a Bronze Leaf

Silver Leaf Member:
($250-$500)

Honorable mention in all NNHC print materials, a plaque on our
“Tree of Life”, a Silver Leaf Member T-Shirt and (2) tickets
to an Annual Gala

Gold Leaf Member:
($500-$750)

Honorable mention in all NNHC print materials, a plaque on our
“Tree of Life”, (6) tickets at an
Annual Gala and an invitation to our Gold/Platinum Donor Appreciation Luncheon

Platinum: ($750 and above)

Honorable mention in all NNHC print materials, a table at an Annual Gala, and (2) invitations to our
Gold/Platinum Donor Appreciation Luncheon
All funds donated are guaranteed to benefit patients receiving care at NNHSC. Most of these individuals are medically underserved Chicago men, women and children who are at-risk for poor health outcomes.

NNHSC is the second largest community health provider
in Chicago, providing comprehensive primary care, social
service and health/nutrition education programs to the medically underserved. As our mission states “…programs will empower individuals to be healthy through educational and skill building.” That is why your donation to this cause is so vital to the success of our “Tree of Life Sponsorship” program.
