Accountant/Internal Auditor

Perform various accounting functions and assists in the detailed preparation of program and center wide budgets and financial reports under the direction of the Director of Finance and Accounting.  Bachelor’s Degree in Accounting and at least 3 years of experience with accounting.  Code: WT/AIA

Advanced Nurse Practitioner

Provides medical care to adult, adolescent and pediatric individuals and families in the Health Center & other facilities.  Plans and implements programs in community health assessment and education. Bachelor’s Degree in Nursing. Currently licensed as an Advanced Practice Nurse in the State of Illinois.  Code: TL/FNP

Assistant Medical Director Administers, directs and coordinates activities of the organization to carry out its objectives in the provision of health care, furtherance of education and research, and participation in community health programs. Is responsible for the direct management and operation of the organization, for the application and implementation of established policies, and for liaison among the governing authority, the medical staff, and the departments of the organization. Graduation from an accredited college or university with graduate degree in an accredited program in Hospital Administration, Public Administration, Business Administration or similar related Health program. A minimum of three (3) years of serving in subordinate administration positions required. Prefer experience in ambulatory health care setting. Code: TL/AMD

Case Manager:
Provides Case Management Services to pregnant and parenting women, infants and children to affect the delivery of health and supportive services in an integrated manner. Maintains an assigned caseload. Conducts monthly or as needed Home Visits to each participant. Completes Individual assessments to determine the level of the client’s health & service needs. Bachelor’s Degree (BSN or other related field in a behavioral science, social service or health-related area). Code: CM/JH

Clinic Generalist

Assumes responsibility for participating in the delivery of care for patients (pediatric and geriatric) under the direct supervision of the Charge Nurse & Team Leader.  Functions as a member of the total health care team with providers and nurse practitioners.  Medical or Nursing Certificate required.  Code MM/CG

Controller

Responsible of administering the plan for the control & operations of the Finance/Accounting Department with particular concentration on grants administration. 3-5 years senior level management. BA degree with CPA Certification.  Code: WT/CTL

Dentist Performs clinical dental services for clients for the City dental clinics which includes examinations and treatment planning, oral cancer screenings, amalgam and composite restorations, pit and fissure sealants, bonded restorations, extractions, root canal therapy, minor oral surgery, periodontal treatments, full and partial denture construction and other required procedures. Requires a current license to practice dentistry. Current DEA and DPS registrations for the purpose of writing prescriptions also required. Code: TL/DT

Family Practice Physician A Family Practice Physician who provides medical care to adult, adolescent and pediatric individuals and families in the Health Center & other facilities, and in the home when appropriate. Plans and implements programs in community health assessment and education. Collaborates with the administration, medical staff and nursing staff of network centers in setting standards for overall care and implementation of these goals. Directly supervised by the Medical Director or other designated physician but works independently as appropriate for training and experience. Graduate of an Accredited Medical School. Code: TL/FPP

Health Information Management Clerk While under the general supervision of the Medical Records Supervisor or designee, responsible for assembling, repairing, retrieving, filing and controlling client medical records. Also responsible for assigning medical record numbers for patient’s subsequent visits. Assists in outlying areas during computer downtime. Special projects or other assignments as designated. High School Diploma or equivalent. Additional training in clerical skills or office management helpful. Code: MM/HIM

Internist

Provides primary medical care services to adult patients in both the ambulatory and in-patient setting using primary care concepts. Collaborates with the Adult Advance Practice Nurses in the preparation and implementation of patient care protocols.  Participates in the program planning of health activities within the Health Center and in the community, as requested. Current Illinois State License, Controlled Substance License & DEA. Completed an Accredited Post-Graduate Training Program in Internal Medicine.  Board Certified or Eligible in Internal Medicine.  Code: TL/INT

Licensed Practical Nurse

Participates in the planning and delivery of health care to patients during clinic sessions under the direction of the RN clinical nurse and Charge Nurse. High School diploma or equivalent. Graduate of an accredited Practical Nurse Program. Successful completion of pharmacology course. Current State of Illinois License. Code: Code MM/LPN

Medical Director Directs, coordinates and is responsible for all medical and related activities. Responsible for verifying credentials of provider staff and recommending their qualifications to the Executive Director & the Board. Affairs Committee of the Board of Directors. Assists in establishing standards of medical care; develops organizational plans to carry out activities within limits of available funds. Plays a primary role in establishing and maintaining a Quality Improvement Program. Advises Executive Director on medical and administrative problems and on questions of policy and public relations as they related to medical practices. Graduate of a medical school approved by the Council of Medical Education and Hospitals of the American Medical Association. Licensed by State to practice medicine. Meet credentialing requirements of affiliated hospitals (Children’s Memorial and NWM Hospital). Must be Board Certified. Code TM/MD

Outreach Worker/ Community Services
(Part-Time)

Conduct outreach activities to identify potential program participants. Knowledge of community agencies, organizations and providers. Works with the Case Management Dept.  High School Diploma or equivalent preferred.  Code: ER/OW

Referral Coordinator
Under general direction of the Director of Nursing, makes referrals for each disciple throughout NNHSC. Participates in the Quality Improvement audit process. Collaborates with the nursing and provider staff regarding the referral process. Communicate frequently with out side agencies. Exhibits professional attitude toward patients, visitors, providers and co-workers. Associate Degree, or Nursing certification. Code: MM/RC

Registered Nurse

Responsible for the planning, coordinating and evaluation of health care services in a specific clinical area.  Provides direct patient care.  Graduate of a accredited school of nursing (RN).  Minimum of 3 years experience.  Must have strong clinical skills and recent experience in Pediatrics, Obstetrics/Gynecology or Medical/Surgical Nursing.  Experience in patient and community health education programming is a plus.  Code: MM/RN

Site Coordinator (LLHC)
Assists in managing and directing the clinical services of the center and collaborates with the Medical Director and Director of Nursing/Clinic Manager in helping to plan, develop and evaluate the services. Provides on-site day-to-day clinical supervision of staff in OB/Gyne, Pediatrics, Internal Medicine and the Lab. Graduate of an Accredited School of Nursing (RN) with minimum of 4 years in an ambulatory care setting. Current RN License in the State of Illinois. Code: MM/SC

Substance Abuse Counselor
Provide substance abuse treatment and counseling. Perform substance abuse assessments, treatment planning, record management, and/or case management for patients referred into treatment for alcohol, drug and/or nicotine abuse/dependency in accordance with DASA Licensing Regulations and ASAM Patient Placement Criteria. Bachelor’s Degree, Master’s preferred and/or formal training in the field of Substance Abuse Treatment, rehabilitation or counseling. Certified Alcohol and other Drug Abuse Counselor (C.A.D.C.) through the Illinois Alcohol and other Drug Abuse Professional Certification Association, Inc. or be board eligible for provisional certification by having an application portfolio board approved by the Illinois Alcohol and other Drug Abuse Professional Certification Assoc., Inc. X-Ray Technician
(Part-Time)

Operates x-ray equipment to make radiographs of designated portions of the body and functions as a member of the total health team under the direction of the Medical Director.   Two years of college and coursework in an Accredited Medical Technologist program is required.  Minimum of 6 months experience in a clinical setting.  Current Accreditation Certificate in the state of Illinois required.  Code: TL/XRT
