Assistant Director of Nursing Responsible for coordinating and evaluating high quality healthcare services in accordance with the NNHSC healthcare guidelines and overseeing the clinical operation of the health Centers. Bachelor’s or Master’s Degree in Health Service Administration, Nursing or equivalent current Illinois State License as Registered Nurse. Three years supervisory experience in a clinical support area. Code: CF/AND

Billing Clerk Primary responsibility is to prepare claims for third party carriers for out-patient and in-house services. Maintain filing system for third party patients, follow-up rejections and record payments. High School Diploma or equivalent. One year experience billing private insurance and Medicare. Code: WT/BT

Chronic Care Coordinator Lead team(s) in development of Diabetes, Asthma, Heart Disease, and Cancer care protocols, keys measures, and standards of care. Establishes program goals and objectives, with the assistance of the Chronic Care Collaborative Team, for chronic disease care and management. Licensed Practical Nurse, Registered Nurse or MA degree in public health or a closely related field. Code: CL/CCC

Director of Development and PR Primary responsibility will be to develop new and sustained support from philanthropic sources to ensure adequate funding for NNHSC to achieve funding goals. Supervise the Public Relations functions and communications efforts. Act as liaison for media relations. BA required. Master’s preferred. 5 years management in ambulatory care or similar setting. Code: BT/DDP

Director of Facilities and Safety Responsible for directing the daily activities of NNHSC facilities with particular focus on maintenance, security, buildings & grounds, pest control, safety and emergency preparedness. The ability to manage and organize functions for the Corporation. Bachelor’s Degree or equivalent to the completion of an engineering vocational program or certification in a technical specialty. 5 - 7 years in health care facility management, including five years management experience.

Director of Nutrition Serves as the chief specialist on Nutrition. Plans, develops and directs the agency nutrition program for the promotion of public health and the conservation of human resources through the dietary aspects of the control of disease. . Minimum four (4) years of progressively responsible professional experience in nutrition, including two years in a consultative or supervisory capacity. MA in Public Health or Nutrition and Dietetics from an Accredited college or university Code: BT/DON

Director of Operations Assist the Executive Director as a responsible member of the Senior Management team. Oversees, directs, administers, and coordinates the integrated operational activities of NNHSC in support of policies, goals and objectives established by the Executive Director. The Director of Operations is expected to act in the absence of the Executive Director. Provide day-to-day leadership and management that mirror the mission, vision and values of NNHSC. Code BMT/DO

Family Nurse Practitioner Provides medical care to adult, adolescent and pediatric individuals and families in the Health Center & other facilities. Plans and implements programs in community health assessment and education. Bachelor’s Degree in Nursing. Currently licensed as an Advanced Practice Nurse in the State of Illinois. Code: BMT/ANP

Family Practice Physician -Provides primary medical care services to pediatric and adult patients in both the ambulatory and in-patient setting using primary care concepts, emphasizing prevention of illness and the promotion of health lifestyles. Current ILL. State License Controlled Substance License & DEA. Code: CL/FP

Internist
Provides primary medical care services to adult patients in both the ambulatory and in-patient setting using primary care concepts, emphasizing prevention of illness and the promotion of health lifestyles. Collaborates with the Adult Advance Practice Nurses in the preparation and implementation of patient care protocols. Provides leadership in Internal Medicine to other health care professionals. Current ILL. State License Controlled Substance License & DEA. Code: CL/IN

Licensed Clinical Social Worker (Bilingual) Maintain active clinic caseload and collaborate with other service programs within the social service department, as well as, other departments of NNHSC. Master’s Degree in Social Work. Current State License and 3 years experience Bilingual required (Spanish). Code: AF/BSW

Obstetrician/Gynecologist

Provides obstetric, gynecological and family planning care in ambulatory and in-patient settings for NNHSC patients. Supervises and may maintain a collaborative practice agreement with Advanced Practice Nurses (Nurse Practitioners and Certified Nurse Midwives) and other mid-level providers. Current ILL. State License Controlled Substance License & DEA. Code: CL/OG

Podiatrist Provides podiatric medical service to patients who in ambulatory capacity. Collaborates with other NNHSC providers in the preparation and implementation in podiatric aspects of health services care. Participates in the program planning of health services at the clinic and in the community as needed. Current Illinois State License, Controlled Substance License & DEA. Code: CL/PD
