Advanced Nurse
Practitioner (FNP)
Provides medical care to adult, adolescent and pediatric individuals and families in the Health Center & other facilities. Plans and implements programs in community health assessment and education. Bachelor’s Degree in Nursing. Currently licensed as an Advanced Practice Nurse in the State of Illinois. Code: BMT/ANP

Charge Nurse, R. N. Responsible for the planning, coordinating and evaluation of health care services in a specific clinical area. Provides direct patient care. Minimum of 3 years experience. Must have strong clinical skills and recent experience in Pediatrics, Obstetrics/Gynecology or Medical/Surgical Nursing. Experience in patient and community health education programming is a plus. Graduate of an accredited school of nursing (RN). Code: AS/RN

Director of
Comprehensive Services
Direct, plan, design and oversee NNHSC’s Comprehensive Services Department including case management services, substance abuse, social work, domestic violence and youth initiatives/drop-in center programs. Will coordinate and integrate social service and mental health programs into the medical healthcare services. Master’s Degree in Social Work and/or Nursing with emphasis in Psychology or LCSW Certification. 5 years of management experience in an ambulatory setting. Code: YC/DCS
Director of Quality Improvement Oversee all current Quality Improvement activities at NNHSC. Manage the process of continuous clinical quality improvement including the Q.I. subcommittee. Directs QI subcommittee meetings. Coordinates, administers and implement actions and processes for Quality Improvement Program. Education: Masters Degree in Health Administration or equivalent degree. Nursing Credential/License. Minimum Experience: At least 5 years experience in ambulatory health care setting. Knowledge and experience of Quality Improvement principles and practices. Excellent written and verbal communication skills. Proficiency in Windows based operating systems. Code: CL/QI

Facility Manager Responsible for directing the daily activities of NNHSC facilities with particular focus on maintenance, security, buildings & grounds and ability to manage and organize functions for the Corporation. Education: Bachelor’s Degree or equivalent to the completion of an engineering vocational program or certification in a technical specialty. Minimum Experience: 5 - 7 years in health care facility management, including two years supervisory experience. Code: WT/FM

Licensed Practical Nurse
Bilingual Preferred

Participates in the planning and delivery of health care to patients during clinic sessions under the direction of the RN clinical nurse and Charge Nurse. High School diploma or equivalent. Graduate of an accredited Practical Nurse Program. Successful completion of pharmacology course. Current State of Illinois License. Code: AS/LPN

Nutritionist

Provides and evaluates WIC nutrition services to individuals, families and community groups. Contributes in the effort to achieve the management and nutrition education objectives set for the fiscal year. LDN or Registered Dietitian Certification preferred. Dietetic training/preferred. Code: AJ/NT

Office Manager Responsible for organizing and coordinating office operations and procedures. Reports to Executive level management and will maintain and oversee overall functionality of office. Special training in office machines/procedures. At least 2 years of previous experience as an Office Manager or 4 years executive level assistant in clinical or similar setting. Experience with grant & proposal typing (also should be able to assist in preparation). Meet or exceed 65 WPM typing speed. Bachelor’s degree required. Code: BMT/OM

Substance Abuse Counselor (CADC) Provide substance abuse treatment and counseling. Perform substance abuse assessments, treatment planning, record management, and/or case management for patients referred into treatment for alcohol, drug and/or nicotine abuse/dependency in accordance with DASA Licensing Regulations and ASAM Patient Placement Criteria. Certified Alcohol and other Drug Abuse Counselor (C.A.D.C.) through the Illinois Alcohol and other Drug Abuse Professional Certification Association, Inc. or be board eligible. Bachelor’s Degree, Master’s preferred and/or formal training in the field of Substance Abuse Treatment, rehabilitation or counseling. Code: MR/SAC

Triage Nurse (RN) - Bilingual Preferred Responsible for triaging patients. Provides some direct patient care. Graduate of an accredited school of nursing (RN). Minimum of 3 years experience. Must have strong clinical skills and recent experience in Pediatrics, Obstetrics/Gynecology or Medical/Surgical Nurse. Experienced in community outpatient health services a plus. Code: AS/TRN
