- Two (2) Check Stubs
- W2 Form
- Medicaid Card
- Food Stamp Card
- Notarized Letter from Employer or Landlord
- Unemployment Notice
Method of Payment:
- Medicaid
- Medicare
- Private Insurance
- Self Pay (based on income and family size)
We base payments on a sliding fee scale.
No one is turned away based on their inability to pay!
Medicaid Application Assistance
Medicaid Presumtive Eligibility (M.P.E.)
- Pregnant women with income at or below 200% of poverty are eligible.
- Coverage begins on application date.
- Applicant does not have to go to Public Aid Office for medical coverage only.
- If applicant needs cash and/or food stamps an appointment will be made at the local DHS office.
All Kids
- Medical coverage for children between 0 - 19 years.
- Income at or below 185% of poverty.
- Application completed on site.
